This is a Mamiya 645 Pro, Super, Left Hand Grip model GL402. 

This grip is basically new as it shows no wear.  This grip is designed for both the Pro and Super Versions.  Only difference is the adapter that comes with it.  You can use the grip on both cameras as long as you get the proper adapter.  This one comes with THE 645 SUPER Adapter.  Using the grip with the adapter will allow you to synch your hot shoe and also allow you to trigger the camera from the grip.  This is one of those MUST HAVE accessories.
