This is a Pentax 645 with a Pentax 645 75mm f/2.8 Lens and a 220 Film Insert.
This Pentax 645, with the 75mm lens, is in excellent condition. Both have been tested and are in good working order. Optics are clean of any scratches, fungus, haze, or separation. Of course, you can use 120 Film in a 220 Insert. Even comes with the INSTRUCTION BOOKLET! May show normal signs of wear.
